Post-Operative Intensive Care

Operational Unit — T.I.P.O.

With the aim of increasing and improving its activities and services, to provide an adequate response to all intensive care and resuscitation needs that may arise within it, the Casa di Cura has activated the Intensive Care Operational Unit. The department, in line with regional and national regulations, is equipped with the required structural and organizational requisites.

The Operational Unit is organized so that the patient undergoing surgery who requires intensive medical and nursing specialist assistance can benefit from it in the most appropriate way and with the adequate timing, from the moment of access until discharge. These are particularly patients whose critical condition does not allow for adequate management in a regular ward, and who may require intensive monitoring, administration of vasoactive drugs, and assisted or controlled artificial ventilation.

Access criteria

The access criteria for patients undergoing surgery refer to the guidelines published by SIAARTI. The clinical appropriateness of access is based on three elements.

1

Reversibility

Reversibility of the acute pathological state.

2

Compensation expectation

Reasonable expectations of returning to compensation an organ or system pathology that may decompensate following a stressful event, such as surgery.

3

Expected benefit

Probability of benefit from the intensive treatment undertaken, also in relation to its burdens.

Clinical conditions that may require hospitalization

Patients undergoing surgical treatment who, due to their particular underlying clinical conditions, require intensive monitoring for at least 12-24 hours and may develop:

Clinical conditions that may require hospitalization in Post-Operative Intensive Care
Scope Clinical conditions
Respiratory
  • Acute respiratory deficits with indication for ventilatory therapy, invasive or non-invasive
  • Acute pulmonary edema
Cardiocirculatory
  • Heart failure of NYHA class III and IV
  • Arrhythmias at risk of hemodynamic instability
  • Hypertensive emergencies
  • Hypovolemic shock
  • Anaphylactic shock
Metabolic
  • Metabolic coma
  • Diabetic ketoacidosis
  • Severe electrolyte imbalances

The presence of the department allows surgeons and orthopedists to perform complex procedures with complete peace of mind, even on patients with increased surgical risk.

Medical-nursing team

The department is characterized by patients with conditions of such severity that they require significant professional, medical, and nursing commitment. The specialized staff hold the required educational qualifications and have proven experience, verified in the field during the integration phase into shifts, in a number adequate to the accreditation requirements of the Emilia-Romagna Region. Under the direction of the Head, they ensure the complete management of the patient, from admission to discharge.

Operational interfaces

To achieve its objectives with maximum efficiency and effectiveness, the department is functionally connected with all Operational Units and Services for the execution of necessary diagnostic and therapeutic procedures. These services and professionals are mostly internal to the Casa di Cura; when external — in the case of superspecialties — they are regulated by specific agreements between the Casa di Cura and the concerned services.

Evaluation and review

Given the delicacy of the tasks entrusted to the department, the Management periodically evaluates processes and outcomes together with the Head, in line with the guidelines of the scientific societies of anesthetists and resuscitators (SIAARTI), with regional and national regulations, and with the corporate quality management system.
